• Please contact us at least two (2) months prior to your desired Baptism date or if you have any questions or need clarification.  All required documents must be completed and submitted to the parish office prior to scheduling a baptism.
  

• REQUIREMENTS

  Required forms can be downloaded in their PDF forms or filled out online from the navigation panel.
• PARENTS
  

  1. Child Birth Certificate
  2. Proof of Adoption (if child is adopted)
  3. Proof at least one parent is Catholic (sacramental certificate)
  4. Completed Infant Baptismal Registration Form
  5. Baptism Preparation Class (Certificate of Attendance if taken at another parish)
  6. Contribution Fee ($50 per child)
• GODPARENTS

  1. Sponsor Eligibility Certificate
  2. Completed Godparent/Sponsor Form
  3. Baptism Preparation Class (Certificate of Attendance if taken at another parish)
  4. Catholic Marriage Certificate (if married)/Confirmation Certificate (if single/unmarried)
